PLP-2 T

Dual GIP and GLP-1. Approved as Mounjaro and Zepbound. Same gastric emptying caution as semaglutide, and then some.

FDA approved medicine

What it isA dual GIP and GLP-1 receptor agonist. Marketed as Mounjaro for type 2 diabetes and Zepbound for weight management and obstructive sleep apnoea.
How it is thought to workActivates both the GIP and GLP-1 receptors, which produces larger effects on glucose and weight than GLP-1 alone in head-to-head trials.
What the evidence actually showsLarge randomised trials across diabetes, obesity and sleep apnoea. The evidence base is substantial and recent.
Regulatory statusThe pharmaceutical product is FDA approved. Research-grade material is not that product.
DosingThe approved labels titrate upward over months from a low starting weekly dose, again to manage gastrointestinal effects. The schedule is published in the linked labels and is a prescribing decision.

Risks and cautions

  • Boxed warning for thyroid C-cell tumours; same contraindications as semaglutide.
  • Pancreatitis, gallbladder disease, and dehydration from GI losses.
  • Loss of lean mass alongside fat.

Why this matters specifically with EDS

Everything in the semaglutide note applies, and tirzepatide's gastrointestinal effects are at least as pronounced. If you have gastroparesis, dysautonomia, or a history of poor oral intake, this is a specialist conversation.
